The best way I can recreate this problem is to switch from currently equipped [Aspects AB and Fragments 1234] to a Loadout with [Aspects BA and Fragments 4321]. The Loadout system has trouble moving both Aspects and Fragments around at the same time. Selecting a Loadout will show you everything that is saved in the Loadouts as well as give you a few options for the Loadout. Select the Loadout that you want to delete.
